The undersigned artist / recording company hereby grants SYBOT, the following authorizations:1. The undersigned hereby expressly authorizes SYBOT to digitally transmit, on an interactive basis, the following sound/video recordings and associated artwork/text:

(Artist/Title)2. The undersigned hereby expressly authorizes SYBOT to duplicate to the SYBOT servers one (1) copy of each of the above-identified sound recordings.
3. Neither SYBOT nor the undersigned recording company shall pay to the other any compensation for the acts authorized herein.
4. The undersigned represents and warrants to SYBOT that the recording company identified below has the right to authorize foreverblue to reproduce, publicly perform and distribute in digital form by an interactive service, copies of the above-listed sound recordings, and that the undersigned has been authorized to execute this Authorization on behalf of the recording company.
5. This authorization may be revoked at any time by the undersigned recording company, provided that the parties agree that such revocation shall become effective five (5) business days after written notice of such revocation is received by SYBOT.
